A recusa do cuidado por paciente em situação de emergência: vivências de profissionais de enfermagem [The refusal of care by patients in emergency situations: nursing professionals’ experiences]

Objetivos: analisar as experiências dos profissionais de enfermagem em relação à recusa do cuidado por paciente em situação de emergência; identificar os sentimentos expressos por eles diante dessa recusa. Método: estudo exploratório, descritivo, de abordagem qualitativa, com 18 profissionais de enfermagem de um Hospital Municipal do Rio de Janeiro. Produção de dados em 2016, através de questionário com perguntas abertas e fechadas. Dados submetidos à análise de conteúdo. Projeto aprovado por Comitê de Ética em pesquisa, protocolo no 52543815.9.0000.5259 Resultados: dessa análise emergiram três categorias temáticas: O paradoxo e o dilema: recusar ou consentir o cuidado; Reações e sentimentos sobre a recusa dos cuidados; A relevância da temática na atualidade. Conclusão: a recusa do cuidado e seu consentimento evidenciam uma estranheza teórica e prática. Provoca um dilema que necessita ser analisado eticamente na medida que o cuidado implica acolher e tratar.
ABSTRACT
Objectives: to examine nursing personnel’s experiences regarding refusal of care by patients in emergency situations; to identify the feelings they express in view of such refusal. Method: in this exploratory, qualitative, descriptive study of 18 nursing professionals at a municipal hospital in Rio de Janeiro, data were produced in 2016 using a questionnaire of open and closed questions, the subjected to content analysis. The project was approved by the research ethics committee (Protocol No. 52543815.9.0000.5259). Results: three thematic categories emerged from the analysis: The paradox and the dilemma: to refuse or consent to care; Reactions and feelings about refusal of care; and the present relevance of the issue. Conclusion: the refusal of, and consent to, care embody a dissonance in theory and practice. This poses a dilemma that needs to be analyzed ethically in that care entails receiving and treating.
RESUMEN
Objetivos: analizar las experiencias de los profesionales de enfermería en relación con la negativa de la atención por parte de pacientes en situación de urgencia; identificar los sentimientos expresados por ellos ante esta negativa. Método: estudio exploratorio, descriptivo, con enfoque cualitativo, junto a 18 profesionales de enfermería de un Hospital Municipal de Río de Janeiro. Producción de datos en 2016, a través de un cuestionario con preguntas abiertas y cerradas. Los datos han sido sometidos al análisis de contenido. El proyecto fue aprobado por el Comité de Ética en Investigación, Protocolo 52543815.9.0000.5259. Resultados: este análisis reveló tres categorías temáticas: la paradoja y el dilema: rechazar o aceptar la atención; reacciones y sentimientos sobre el rechazo a la atención; la relevancia de la temática en la actualidad. Conclusión: el rechazo a la atención y su consentimiento muestran una extrañeza teórica y práctica. Provoca un dilema que debe ser considerado éticamente puesto que el cuidado conlleva a acoger y dar tratamiento.
