Características dos pacientes críticos que apresentaram hipoglicemia: uso seguro da infusão continua de insulina [Characteristics of critical patients with hypoglycemia: safe use of continuous infusion of insulin]

Pesquisa com o objetivo de descrever as características dos pacientes críticos com hipoglicemia grave. Estudo transversal, prospectivo, com população de 42 pacientes do pós-operatório de cirurgia cardíaca que receberam insulina intravenosa, dos quais 15 apresentaram hipoglicemia. As características com associação positiva e significância estatística para a ocorrência de hipoglicemia foram o hematócrito baixo (p=0.03852) e a cirurgia de emergência (p=0,02884).Outras características somente apresentaram associação positiva, como a insuficiência renal com tratamento dialítico, a PaO2 ≥90mmHg e o uso de vasoaminas (OR>2). Conclue-se que o enfermeiro deve levar em consideração as características que influenciam a glicemia para que evite erros na titulação das doses de insulina administrada.
ABSTRACT
This cross-sectional, prospective study to describe the characteristics of critically ill patients with severe hypoglycemia was conducted in 2014 at a federal public hospital, with 42 post-operative cardiac surgery patients receiving intravenous insulin, 15 of whom presented hypoglycemia. Characteristics with positive associations and statistical significance for occurrence of hypoglycemia were low hematocrit (p=0.03852) and emergency surgery (p=0.02884). Other features showing positive associations only included kidney failure treated with dialysis, PaO2≥90mmHg, and use of vasoactive amine (OR>2). It was concluded that nurses must take account of characteristics that influence glycemia so as to avoid errors in titration of insulin doses for administration.
RESUMEN
Investigación con el objetivo de describir las características de los pacientes críticos que presentaban hipoglucemia grave. Estudio transversal, realizado en 2014, en hospital público federal, obtenido a partir de 42 pacientes del post operatorio de cirugía cardiaca que recibieron insulina intravenosa; entre ellos, 15 (35,71%) tenían hipoglucemia. Las características con asociación positiva y la significación estadística para la ocurrencia de hipoglucemia fueron el hematocrito bajo (p = 0,03852) y la cirugía de emergencia (p = 0,02884). Otras características sólo presentaron asociación positiva, como la insuficiencia renal con tratamiento dialítico, la PaO2 ≥90mmHg y el empleo de vasoaminas (OR>2). Se concluye que el enfermero debe llevar en consideración las características que influencian la glucemia para evitar errores en la titulación de las dosis de insulina administrada.
