Ocorrência de eventos adversos como indicadores de qualidade assistencial em unidade de terapia intensiva [Occurrence of adverse events as indicators of quality of care in an intensive care unit]

Estudo descritivo, quantitativo, desenvolvido em unidade de terapia intensiva de um hospital público do sul do país, o qual objetivou avaliar a qualidade assistencial e segurança do paciente por meio da ocorrência de eventos adversos. A coleta de dados foi realizada em agosto e setembro de 2012, por meio de um instrumento de notificação e observações de fatores de risco para eventos adversos. Das notificações realizadas, 17 (54,8%) foram para perda de sonda gastroenteral, 8 (25,8%) para úlcera por pressão, 3 (9,7%) para perda de cateter venoso central e 3 (9,7%) para extubação acidental. Dos riscos observados, 74 (54%) foram para úlcera por pressão, 32 (23,4%) para perda de sonda gastroenteral, 22 (16%) para perda de cateter venoso central e 9 (6,6%) para extubação acidental. Evidenciou-se a importância no monitoramento dos eventos por meio da aplicação de indicadores de qualidade com o intuito de melhoria da segurança e assistência ao paciente.
ABSTRACT
This descriptive, quantitative study in the intensive care unit of a hospital in southern Brazil, evaluated quality of care and patient safety by occurrence of adverse events. Data were collected in August and September 2012 using an instrument for notification of, and observations on, risk factors for adverse events. Of total notifications, 17 (54.8%) were for loss of gastrointestinal probe, 8 (25.8%) for pressure ulcer, 3 (9.7%) for loss of central venous catheter and 3 (9.7%) for accidental extubation. Of the risks observed, 74 (54%) were for pressure ulcer, 32 (23.4%) for loss of gastrointestinal probe, 22 (16%) for loss of central venous catheter and 9 (6.6%) for accidental extubation. This highlighted the importance of monitoring for events by applying quality indicators in order to improve patient safety and care.
RESUMEN
Estudio descriptivo cuantitativo, realizado en una unidad de cuidados intensivos de un hospital en el sur del país. Tuvo como objetivo evaluar la calidad de la atención y seguridad del paciente a través del suceso de eventos adversos. La recolección de datos tuvo lugar en agosto y septiembre de 2012, a través de un instrumento de notificación y observaciones de factores de riesgo para eventos adversos. De las notificaciones realizadas, 17 (54,8%) fueron sobre pérdida de sonda gastroenteral, 8 (25,8%) sobre úlcera por presión, 3 (9,7%) sobre pérdida de catéter venoso central y 3 (9,7%) sobre extubación no programada. Se evidenció la importancia de monitorear los sucesos a través de la aplicación de indicadores de calidad con el objetivo de mejorar la atención y seguridad del paciente.
