Integralidade como uma dimensão das práticas de atenção à saúde da criança: uma revisão bibliográfica [Comprehensiveness as a dimension of child healthcare practices: a literature review]

O estudo objetivou analisar a prática da integralidade em publicações que abordam a saúde da criança na atenção básica. Realizou-se busca nas bases de dados eletrônicas LILACS e SCIELO, no período de 1984 a 2009. A amostra foi composta por 14 artigos analisados quanto à integralidade como atributo das práticas dos profissionais; formas de organização dos serviços; e políticas de saúde. Constatou-se que as ações preconizadas pelo Programa de Assistência Integral à Saúde da Criança e Atenção Integrada às Doenças Prevalentes na Infância ocorriam de forma pontual, fragmentada e centrada no modelo biológico, que privilegia práticas curativas. Também as competências da unidade básica na rede hierarquizada de saúde ainda não se concretizam no sentido de articulação da atenção primária aos demais níveis de atenção. Aponta-se a capacitação dos profissionais e a reorganização dos serviços como fundamentais para garantir a integralidade da atenção à saúde da criança.
ABSTRACT
This study aimed to examine for practical application of the principle of comprehensiveness in publications on children’s health in primary care. The search was conducted in the Lilacs and Scielo electronic databases for the period 1984 to 2009. The sample comprised 14 articles on comprehensiveness as an attribute of health personnel’s practice, forms of service organization, and health care policies. It was found that the actions prescribed by the Program of Comprehensive Child Health Care and Integrated Management of Childhood Illness occurred only occasionally, and in a fragmented and biologically-centered manner, favoring curative treatment. Also the competences of the primary health care clinic in the hierarchical health system are not fulfilled in that primary care is not coordinated with other levels of care. Capacity building and service reorganization are identified as fundamental to assuring comprehensive children’s health care.
RESUMEN
Este estudio tuvo como objetivo analizar la práctica de la integralidad en publicaciones que enfocan La salud infantil en la atención primaria. Se realizó búsqueda en las bases de datos electrónicas LILACS y SCIELO de 1984 a 2009. Muestra de 14 artículos fue seleccionada y analizada, teniendo la integralidad como atributo de La práctica profesional, formas de organización de servicios y políticas de salud. Se constató que las acciones Del Programa de Atención Integral a la Salud Infantil y Manejo Integrado de Enfermedades de la Infancia se presentaron de forma reducida, fragmentada y con foco en el modelo biológico, que se centra en las prácticas curativas. Las competencias de la unidad básica de la red jerárquica de salud no se han materializado en el sentido de articulación de la atención primaria a otros niveles de atención. E estudio mostró como clave para garantizar la atención integral a la salud del niño, la capacitación profesional y la reorganización de los servicios.
