Manejo de eventos adversos pós-vacinação pela equipe de enfermagem: desafios para o cuidado [Management adverse events following immunization for nursing team: care challenges]

O manejo de eventos adversos pós-vacinação (EAPV) constitui um aspecto importante da utilização segura de vacinas. Este trabalho teve como objetivo analisar como a equipe de enfermagem de um centro de especialidades desenvolve as práticas de cuidado relacionadas ao manejo de EAPV leves, moderados e graves entre crianças de até um ano de idade. Trata-se de um estudo exploratório, descritivo, de abordagem qualitativa. A coleta de dados foi realizada por meio de observação participante, análise documental e entrevista estruturada com sete trabalhadores de enfermagem de um serviço de referência em vacinação, em Macaé/RJ, em junho de 2010. Para o tratamento dos dados realizou-se análise categorial. Os resultados apontam: subnotificação; fragilidades nas ações de vigilância epidemiológica e uma atuação restrita da equipe de enfermagem no manejo adequado de EAPV. Estes achados constituem importantes desafios para a utilização segura de vacinas e para o desenvolvimento do cuidado de enfermagem na prática vacinal.
ABSTRACT
The proper management of adverse events following immunization (EAPV) is an important aspect of safe use of vaccines. This study analyzed how be nursing staff from a health care center develops their care practices related to management of mild, moderate and severe EAPV among children under one year old. It is an exploratory, descriptive, qualitative approach study. Data was gathered in with 7 nursing from a reference vaccination service by participant observation, document analysis and structured interviews in Macaé/RJ, in june 2010. The categorical analysis was used to treat. The results suggest: inappropriate epidemiological surveillance and a restricted role of the nursing staff in the management of EAPV. Such challenges have hindered the development of care by nursing staff immunization practice.
RESUMEN
La gestión de eventos adversos posteriores a la vacunación (EAPV) constituye un aspecto importante del uso seguro de las vacunas. Este estudio tuvo como objetivo analizar cómo el personal de enfermería de un centro de atención especializada desarrolla prácticas relacionadas con la gestión de EAPV leves, moderados y severos en niños menores de un año. Este es un estudio exploratorio, descriptivo, de enfoque cualitativo. La recolección de datos se llevó a cabo a través de observación participante, análisis de documentos y entrevistas estructuradas con siete trabajadores de enfermería de un servicio de referencia en vacunación, en Macaé/RJBrasil, en junio de 2010. Para el tratamiento de los datos se utilizó el análisis de categoría. Los resultados sugieren: subregistro, deficiencias en la vigilancia y un papel limitado del personal de enfermería en el manejo adecuado de los EAPV. Estos hallazgos son importantes desafíos para el uso seguro de vacunas y el desarrollo de los cuidados de enfermería en la práctica de la vacunación.
