Objetivo: desvelar as práticas discursivas e sociais de profissionais de saúde e de familiares cuidadores sobre a alta hospitalar de crianças com necessidades especiais de saúde. Método: pesquisa qualitativa, realizada em 2013, junto a seis profissionais de saúde e 11 familiares cuidadores, na unidade de pacientes internos de um Hospital Universitário Federal Pediátrico no município do Rio de Janeiro, após aprovação pelo Comitê de Ética em Pesquisa da instituição (CAAE: 01504512.2.0000.5264; Parecer n.º 39272). Conjugaram-se as técnicas de entrevista semiestruturada com Dinâmicas de Criatividade e Sensibilidade do Método Criativo e Sensível. Aplicou-se a Análise Crítica de Discurso. Resultados: revelaram-se três tipos de alta: a clínico-administrativa, a procedimental e a social. Há uma fragmentação dos papéis de cada profissional na alta. Conclusão: para a continuidade do cuidado em casa com autonomia, segurança e qualidade, faz-se necessária a construção de um processo de alta interdisciplinar, dialógico e pautado na integralidade do cuidado.
ABSTRACT
Objectives: to unveil discursive and social practices of health professionals and family caregivers about hospital discharge of children with special health care needs. Method: qualitative study in 2013, with six health professionals and eleven family caregivers, at the inpatient unit of a federal pediatric hospital in Rio de Janeiro, after approval by the institution’s research ethics committee (CAAE: 01504512.2.0000.5264; Report No. 39272). Semi-structured interviews were used together with dynamics of the Creative-Sensitive Method. Critical discourse analysis was applied. Results: discharge was revealed to comprise three types: clinical-administrative, procedural and social. There was found to be fragmentation among professionals’ role in discharge. Conclusion: in order for there to be continuity in care at home with autonomy, security and quality, it is necessary to construct an discharge process that is interdisciplinary, dialogic and guided by the need for comprehensive care.

Objetivo: revelar las prácticas discursivas y sociales de los profesionales de salud y los familiares cuidadores sobre el alta hospitalaria de niños con necesidades especiales de salud. Método: Investigación cualitativa, llevada a cabo en 2103, junto a seis profesionales salud y once familiares cuidadores, en la unidad de pacientes internos de un Hospital Universitario Federal Pediátrico en Río de Janeiro, tras la aprobación del Comité de Ética en Investigación de la institución (CAAE: 01504512.2.0000.5264; Dictamen n.º 39272). Se conjugaron las técnicas de entrevista semiestructurada con dinámicas de creatividad y sensibilidad del Método Creativo y Sensible. Se aplicó el Análisis Crítico del Discurso. Resultados: se revelaron tres tipos de alta: la clínico-administrativa, la de procedimiento y la social. Hay una fragmentación de las funciones de cada profesional en el alta. Conclusión: Para la continuidad de la atención en casa con autonomía, seguridad y calidad, es necesaria la construcción de un proceso de alta interdisciplinario, dialógico y guiado por la atención integral.
