Introduction: Hypertension is a public health problem and may be associated with low muscle strength. We did not identify studies associating relative muscle strength (RMS) with hypertension in a population of Germanic origin in Brazil. Objective: To verify the association between FMR and SAH in adults from a German colonized city in southern Brazil. Methods: Cross-sectional, population-based study with 1,755 aged 20 to 59 years from Pomerode/SC, participants of the Study of Health in Pomerode - SHIP-Brazil. Hypertension was considered if blood pressure ≥140 x 90 mmHg and/or use of antihypertensive medication. The RMS was calculated by dividing handgrip strength (kg by dynamometry) and body mass (kg/m2). Low RMS were considered if participants qere included in the lower quartile. The association between RMS and hypertension, stratified by sex, was estimated by crude Poisson regression and adjusted for sociodemographic, lifestyle and clinical variables. Results: The median of RMS was lower among women (0.36 IQ: 0.29-0.42 vs 0.53 IQ:0.45-0.61; p <0.001). The proportion of hypertension was higher among men (22.4 vs 19.9; p<0.001). Median of RMS was lower in people with hypertension among men (0.48 vs 0.55; p<0.05) and women (0.31 vs 0.37; p<0.05). After adjustment for all variables, the low RMS was associated with hypertension among men (PR=1.27; 95%CI:1.16-1.39) and women (PR=1.43; 95%CI:1.31-1.57). Conclusions: Low RMS was associated with the presence of hypertension in adults of both sexes.
