Telemonitoring in cardiovascular care: experiences in an extension project
Videomonitoring has emerged as a relevant strategy in the care of individuals with cardiovascular diseases, particularly in the follow-up of patients with heart failure (HF), a chronic condition associated with high morbidity, mortality, and frequent hospital readmissions. Within the context of university extension, the use of this technology enables expanded access to health education, strengthens the bond between university and community, and promotes self-care in the post-discharge period. This report describes the experience of a university extension initiative that used videomonitoring as an educational and remote follow-up strategy for patients with heart failure. This is an experience report of an extension initiative developed by faculty members, nurses, and undergraduate nursing students from a public university in Minas Gerais, Brazil, in partnership with patients with HF discharged from a cardiology service. The activities included the development of educational materials, structured videomonitoring follow-up at different time points after hospital discharge, and dialogical health education focused on self-care, considering participants’ needs, lived experiences, and prior knowledge. The experience demonstrated high engagement of participants and their families, strengthening of bonds with the extension team, improved understanding of the disease, and greater adherence to self-care practices. Videomonitoring proved to be a powerful tool for fostering closer university-community interaction and promoting more humanized and accessible care, despite challenges such as technological limitations and irregular adherence among some participants. The extension initiative demonstrated that videomonitoring in cardiovascular care contributes to health education, strengthening of self-care, and integration between university and community. Initiatives of this nature reaffirm the social role of university extension and highlight the potential of health technologies as instruments for social transformation and qualification of care.
