Mortalidade neonatal precoce em recém-nascido com peso de nascimento menor ou igual a 1500 g: fatores de risco e prevenção

Resumo

Em uma perspectiva mundial, nascem em torno de 15 milhões de RN prematuros (<37 semanas) por ano, com crescente aumento dessa taxa nas duas últimas décadas em quase todos os países que dispõem de dados confiáveis. As complicações do parto prematuro são as principais causas e o maior fator de risco para morte neonatal. Embora a mortalidade neonatal tenha diminuído nestes últimos anos, ainda se mantém como um importante problema de saúde pública mundial. Comparados com os recém-nascidos a termo, os prematuros têm maior instabilidade térmica, desconforto respiratório, infecções, apneia, hipoglicemia, icterícia, convulsões, dificuldade de iniciar dieta, e enterocolite necrotizante. A taxa de mortalidade neonatal é inversamente proporcional à idade gestacional e ao peso ao nascimento. O objetivo deste artigo é chamar atenção para os fatores de risco e sua prevenção. O risco da mortalidade neonatal precoce nos prematuros com peso ao nascer ≤ 1500 gramas está associado com vários fatores maternos, obstétricos e neonatais, alguns deles evitáveis. São necessárias mais campanhas para acompanhamento pré-natal e que abordem o risco da gestação em adolescentes, assim como a estimulação à formação continuada e a introdução das novas diretrizes de reanimação neonatal nos cursos de medicina e enfermagem.
